If you prefer to use the downloadable forms, here are the steps: 1) locate and save the appropriate form(s) to your desktop; 2) open the file and fill in the information using the fields provided; 3) print and verify application is complete before mailing. Forms

|
Fingerprints: Any applicant who does not hold a
valid Washington certificate at the time of application must be fingerprinted for a state
and national background check. We urge applicants to submit fingerprints as soon as possible.
To submit fingerprints electronically in person (no inked card is required,
and the process is often much faster) contact an ESD to arrange for scanning
of fingerprints.
If you are unable to come in person, you can request
fingerprint cards by sending an email with your name and mailing address to
cert@k12.wa.us.
Fee Payment: Fees are not refundable. Please ensure you select and submit the correct application as the OSPI $33 fee will not apply toward a future or corrected application and will need to be paid with each application submitted. Refund Policy
