Fresh Fruit & Vegetable Program – Application Packet

The application for school year 2013–2014 is now available.

Deadline: The application and forms must be submitted by May 10, 2013.

Eligibility: Eligible schools are invited to apply for participation on the Fresh Fruit and Vegetable Program (FFVP) for school year 2013-2014. Funds are awarded solely to the highest free and reduced-price percentage schools that apply. Schools with 50 percent or more free and reduced-price eligible students based on the October 2012 building data are eligible to apply for participation. This information is located in OSPI Bulletin B003-13 and its attachment, Free and Reduced-Price Eligibility.

The following forms must be completed and submitted by May 10, 2013.

Submission: You may scan and email, fax, or mail the documents. If mailing, the postmark cannot be later than the deadline, May 10, 2013.
